Think about it, man, have a game where you manage a theme park/mall/restaurant/whatever the fuck and slowly as you build it up your customers keep coming back fatter. Open multiple locations and raise the BMI of an entire city. That’s my shower thought suggestion for you goodbye.

Not to the same extent as what you’re imagining, but parvinjaan’s A Piece of Cake! does have a similar idea or raising the entire city’s BMI (and breast sizes).
That being said, a Roller Coaster Tycoon-esque game of designing an area to purposefully fatten it’s inhabitants does sound amazing. A lot of work, but it would be a great idea!
Thats a genre of game I would love to see further developed, PieceOfCake is great and comes the closest I guess. Civic Waistline Expansion Project is similar like a Simcity managing game: Civic Waistline Expansion Initiative by Lovelessguy
I am probably the biggest fan of strategy games in this entire page and I could easily make a mod like that like I have done for other game mechanics in the past. Biggest issue is just that they are mostly spreadsheet simulators so there’s no in-game “reward” unless you’re really imaginative with seeing stats go up… which the average map painter kinda is, honestly.

I was already making a weight gain city builder game for months, I just don’t know when it will be enough for public.
There’s a little of this is Second Helping, with the dating game slowly morphing into [minor spoilers] a retail simulator where you can open a bunch of doughnut shop franchises, teaming up with the evil masterminds behind the franchise chain (as essentially a low level minion) to gradually elevate the BMI of both the town(s) you’re active in and the country as a whole.
